Update to Kansas UCCC High LTV Notice and Kansas UCCC Election Addendum to Note

DocMagic is updating the form selection logic for the Kansas UCCC High LTV Notice and Kansas UCCC Election Addendum to Note.  DocMagic is updating the logic to accommodate additional data points in the LTV calculation.
